    Hi everyone! We are starting to onboard new vendors who offer AI capabilities. With that said, I need to create an AI Questionnaire for these vendors to answer. I know Venminder has their AI questionnaire template, which is very helpful. I am curious to know if others use an AI Questionnaire and what questions they include. All insight is welcome. Thank you in advance!

    We added an AI section to our existing questionnaire. These are the questions we are asking:

    4. Artificial intelligence (AI)
    4.1 
    Does the product(s)/service(s) your company provides to us leverage Artificial Intelligence?
    If Yes to "Does the product(s)/service(s) your company provides to us leverage Artificial Intelligence?" then ask
    4.1.1 
    Is the AI functionality Opt-in/Opt-out/Mandatory?
    4.1.2 
    Is the AI model and open or closed model?
    4.1.3 
    Does the AI system collect data from public/unverified sources?
    4.1.4 
    Is this AI system considered a "high-risk" AI system as defined by the Colorado Consumer Protections for Artificial Intelligence (SB24-205)?
    **A "high-risk" AI system is defined as any system that, when deployed, makes - or is a substantial factor in making - a "consequential decision"; namely, a decision that has a material effect on the provision or cost of:
    i. education enrollment or an education opportunity,
    ii. employment or an employment opportunity,
    iii. a financial or lending service,
    iv. an essential government service,
    v. health care services,
    vi. housing,
    vii. insurance, or
    viii. a legal service.
