While we don't have anything that would work for your specific request, it would be fairly simple to build a spreadsheet to capture that data
I would suggest, however, making sure a few data points are included.
- Referencing contract number
- Contract expiration date
- SLA description
- SLA targets with upper and lower acceptable limits
- Date (or period) SLA was not met
- SLA penalty (include if it is a rebate, a discount, etc.)
- Report or data source supporting SLA performance
If you are using a manual tracking process such as a spreadsheet, make sure you have a mechanism to track SLA performance over time.
